Based on author Wally Lamb’s award-winning novel, HBO’s I Know This Much Is True follows the story of two brothers who demonstrate that love knows no bounds and can sometimes surprise us.

On I Know This Much Is True Season 1 Episode 1, “Episode One,” viewers get introduced to identical twin brothers Thomas and Dominick Birdsey — both of whom are played by Mark Ruffalo. The two have to come together in support when Thomas suffers a violent public breakdown.

After years of dealing with his brother’s schizophrenia, Dominick now has to take a more public stance. Their bond has been strenuous at best but now he knows that Thomas needs his unwavering support.

How will both of these brothers’ live change as Thomas’ mental illness take further hold of his life? The show leaves us questioning how far we are willing to go for the ones we love.

The series is directed by Derek Cianfrance. And boasts a star-studded cast including Melissa Leo, Rosie O’Donnell, Juliette Lewis, Archie Panjabi, and Kathryn Hahn.

Episode Description: After paranoid schizophrenic Thomas Birdsey has a violent public breakdown, Dominick Birdsey finds himself stepping up to defend his identical twin brother in unexpected ways. As he navigates the fallout of Thomas’ actions, Dominick reflects on their childhood growing up under the tyrannical rule of their volatile stepfather, Ray, and their persistent desire to know the identity of their biological father. Dominick crosses paths with the prickly Nedra Frank as he attempts to have his grandfather’s manuscript translated from Italian into English as a gift for his ailing mother.
